Located upstairs inside the Creekside Plaza
Our practice is open: Mon., *Tues. & Thurs. ~ 8:00am - 5:00pm w/ lunch from 12:00pm - 1:00pm and Fri. ~ 8:00am - 3:00pm. {*4th week of each month ~ closed Tues. & open Wed. instead due to Dr. Moulton teaching @ the ISU Residency Clinic in Pocatello.}
